GENERAL SUMMARY
To assist in the observation, supervision and monitoring of clients in residence.


MINIMUM QUALIFICATIONS
High School Diploma or equivalent. Ability to follow oral and written instructions. Must have a valid Florida Driver's License with 6 points or less in the last 5 years.


PHYSICAL REQUIREMENTS
• Must be in good physical condition with no restrictions of movement.
• Must be able to apply Therapeutic Effective Aggression Management (TEAM) Techniques without limitations.


ESSENTIAL JOB FUNCTIONS
1.0 Guides and participates in daily program activities for new and existing clients in residence.
1.1 Facilitates and/or participates in recreational and therapeutic activities as indicated and/or directed.
1.2 Participates in education, lecture and group discussions (AA, NA, community meetings, etc.) with clients in residence.
1.3 Assists with written assignments and individual goal setting activities with clients.
1.4 Readily develops and establishes therapeutic relationships with clients.
1.5 Effectively interacts with client's family and significant others.
1.6 Is familiar with assigned client's individual treatment plan goals.
1.7 Shift report meetings are attended 100% of the time.
1.8 Room checks are conducted according to facility protocol and procedure.
1.9 Personal hygiene supplies are monitored and promptly replaced, as needed.
1.10 Supervises clients in housekeeping activities; ensures that client area and rooms are neat and orderly at all times.

2.0 Prepares written documentation and record keeping for assigned clients.
2.1 Chart entries are factual and concise and documented using the Dimension format.
2.2 All entries made in the client's medical record are in correct sequence and according to unit protocol and procedure.
2.3 Documentation is neat, legible and in accordance with accepted charting standards.

3.0 Assists with new admissions to the facility.
3.1 New charts are assembled properly.
3.2 Checks for completeness of paperwork.
3.3 Obtains as much general information from the client and family as possible.
3.4 Client orientation to facility is completed in a timely manner according to protocol and procedure.
3.5 Valuables are secured and recorded according to unit protocol and procedure.
3.6 Client's person and belongings are checked according to unit protocol and procedure.
3.7 Client rights and responsibilities are effectively communicated upon admission.

4.0 Accompanies and/or transports clients to and from special procedures outside the Agency and to other facilities.
4.1 Verbalizes, understands and adheres to Agency safety practices and transportation policy and procedures.
4.2 Maintains current driver's license.

5.0 Performs other duties as requested.
5.1 Miscellaneous duties are performed according to instruction.
