GENERAL SUMMARY
Provides psychosocial evaluations, treatment planning and individual and multi-client counseling activities for assigned clients. Maintains clinical and administrative documentation for assigned clients as prescribed by Agency Policy and Procedure and by governing law.


MINIMUM QUALIFICATIONS
Master's degree from an accredited university or college with a major in counseling, social work, psychology, nursing, rehabilitation, special education, health education, or a related human services field. In addition, the practitioner must have training in the treatment of mental health disorders, human growth and development, evaluations, assessments, treatment planning, basic counseling and behavioral management interventions, case management, documentation, psychopharmacology, abuse regulations, patient rights and special clinical circumstances such as emergencies, suicide, and out-of-control behavior.


PHYSICAL REQUIREMENTS
• Must be in good physical condition with no restrictions of movement.
• Must be able to apply Therapeutic Effective Aggression Management (TEAM) Techniques without limitations.


ESSENTIAL JOB FUNCTIONS
1.0 Engages in hours of direct client services for assigned clients at the rate determined to meet contractual and clinical obligations.
1.1 Hours may vary depending on contractual and programmatic need, but shall be at least 26 hours per week.
1.2 Therapists will be informed of their productivity expectations upon hire, at the beginning of each fiscal year, and at any time requirements may change.

2.0 Conducts biopsychosocial evaluations for new and existing clients as assigned.
2.1 Evaluations are performed in accordance with the Agency's protocol for conducting psychosocial evaluations and its procedures relative to frequency and timeliness for the occurrence of this activity.

3.0 Develops and maintains a written treatment plan for each client.
3.1 Written treatment plans are developed with as much participation from the client as possible and reasonable.
3.2 Treatment Plans are maintained in accordance with the Agency's protocol and procedures for frequency and timeliness.
3.3 Treatment plans are appropriately signed and properly credentialed.
3.4 Treatment plan reviews are conducted at a minimum of every 6 months.
4.0 Conducts individual, multi-client and family counseling.
4.1 Individual and multi-client counseling activities are performed in accordance with the Agency's standards for the use of various counseling modalities, as well as frequency and length of counseling sessions.

5.0 Practices a brief, solution focused counseling strategy in the home, school, and community settings.
5.1 To the extent that it is clinically appropriate, counseling activities are provided within a brief, solution focused framework.

6.0 Prepares written documentation for each service conducted.
6.1 Written documentation for each service conducted is performed in accordance with the Agency's standards for service documentation in the client's medical record.
6.2 Service Activity Logs (SALs) are turned in as required by management and with copies of notes attached.
6.3 Clinical records are accurate, individualized, and charting is completed on a daily basis.
6.4 Ensures that each service is medically necessary and approved by the LPHA, for services billed as TBOS, ensures that a certification form is present and signed by LPHA.
7.0 Completes all state and Agency mandated outcome documents.
7.1 Completes state outcome measures (PB2) upon admission and discharge and quarterly for as long as the client remains in the program or as assigned.
7.2 Completes state outcome measures (CFARS) upon admission and discharge and every 6 months for as long as the client remains in the program or as assigned.

8.0 Maintains a current knowledge of psychotherapy and counseling strategies.
8.1 Each counselor is expected to maintain a sufficiently broad range of clinical expertise so as to provide a minimum of a thorough assessment or emergency counseling to most clients seeking services at New Horizons.
8.2 Each counselor is expected to develop at least one area of clinical expertise.

9.0 Maintains a good working relationship with referral sources in the agency and community at large.
9.1 Referral sources are contacted regarding referrals made and questions they may have regarding status of a referred client/intake procedures.

10.0 Performs other duties as requested.
10.1 Miscellaneous duties are performed according to instruction.
