GENERAL SUMMARY
Provides problem identification, prevention services and/or referral services for youth that have used tobacco, alcohol or illicit drugs, in assigned school location. Prevention services will be provided in the classroom, individually, groups and community presentations in compliance with evidence-based services model.


MINIMUM QUALIFICATIONS
Minimum of a Bachelor's Degree from an accredited university. Experience with children facing substance abuse and/or mental health issues preferred. Valid and current Florida Driver's License required with no more than 6 points in the past 3 years.
or
Possess and maintain a certification from the Florida Certification Board and have 5 years of experience with children facing substance abuse and/or mental health issues. Written acceptance for Certification in lieu of a Bachelor's Degree must be obtained from the school administration and kept in personnel file.


PHYSICAL REQUIREMENTS
• Must be in good physical condition with no restrictions of movement.
• Must be able to apply Therapeutic Effective Aggression Management (TEAM) Techniques without limitations.

 

ESSENTIAL JOB FUNCTIONS
1.0 Provides individual and multi-person substance abuse prevention services at the rate of 120 hours per month.
1.1 Prevention services are performed in accordance with the expectations of CSCSLC/CSC Okeechobee/SAMH and any additional entity which provides funding.
1.2 Prevention services are provided in compliance with licensing standards and NHTC standards which apply to prevention.
1.3 Prevention services will be provided in a format which will achieve outcome measures outlined in funded and contracted services.

2.0 Maintains accurate and current case records.
2.1 Level I activities will be documented on the appropriate form (Activity Log) as directed by licensing standards and NHTC standards which apply to prevention.
2.2 Level II documentation will be completed in a chart format and maintained in accordance with the licensing standards and NHTC standards which apply to prevention.
2.3 Documentation of all services and activities will be completed within 24 hours of when the service or activity was delivered.
2.4 Charts will be closed and discharge documentation completed within 5 days of the client completing services, withdrawing from services, parents withdrawing from services, relocation, expulsion or non-compliance.

3.0 Prepares monthly and weekly reports as required by the Agency and/or funding sources.
3.1 Service Activity Logs are to be submitted no less than twice per week.
3.2 Monthly reports are due no later than the 2nd working day of each month.
3.3 Follow-up phone surveys will be completed on a monthly yet rotational basis.
3.4 Waiting Lists/Referral Logs are to be submitted by the first of each month.
3.5 All summer camp documentation will be filed in the summer camp binder daily.

4.0 Performs other duties as requested.
4.1 Other duties such as Red Ribbon Week, health fair representation, summer camp staff activities and community presentations will be coordinated throughout the year according to the schedule and instruction of the Program Manager. Other duties will include working in other NHTC locations (i.e. the SAP office, summer camp sites, or other approved NHTC programs which require your expertise).
