Provides peer support services to clients as a member of the Care Coordination Team for clients utilizing the highest level of services.


Certified as a Peer Specialist preferred or willing to be certified.   Valid and current Florida Driver's License required with no more than 6 points in the past 5 years.   


Must be in good physical condition with no restrictions of movement. 
Must be able to apply Therapeutic Effective Aggression Management (CARE) Techniques without limitations.


1.0  Provides peer support services either in office or in community for an assigned group of clients identified by the MCRT Team Leader.  Provide individual supportive coaching and monitoring of symptom management.  Advocates, supports and provides education to clients, families, and staff regarding client rights and preferences for care coordination, crisis resolution, and aftermath from a crisis response.
1.1 Models and provides side-by-side growth to encourage client achievement of treatment.  Offers Peer Counseling as appropriate and as documented in the client record.
1.2 Participates in gathering information for assessments and outcomes (e.g., onset, course and effect of illness, past treatment and responses, triggers, and risk behaviors), physical health and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living (e.g., self-care, living situation, nutrition, money management); and family structure and relationships.
1.3 Documents client progress to maintain a permanent record of client activity according to established methods and procedures.
1.4 Participates in scheduled staff organizational meetings and care coordination review meetings.
1.5 Participates in linking, supporting, and providing mental health and substance use and treatment services.
2.0 Assists the MCRT with activities for assigned clients to include:
2.1 Direct service activities are performed in accordance with Agency standards and DCF contract provisions for the delivery of services.
2.2 Activities conducted are reflective of the services specified in the written care plan.
2.3 Consumers are consistently observed for significant social, behavioral, emotional or physical problems which, when found to be present, are communicated to the team members.
2.4 Advocate, model, and provide emotional support to clients and promote self-advocacy. 
3.0 Prepares written documentation for each service conducted.

3.1 Written documentation for each service conducted is performed in accordance with the Agency's standards for service documentation in the client's medical record.
3.2 Medical records accurately reflect the nature of service activities provided, timeliness of services and outcome.

4.0 Provides transportation to consumers as needed.
4.1 Verbalizes, understands and adheres to Agency transportation policy and procedures.

4.2 Maintains current and valid driver's license. 
5.0 Performs other duties as requested.
5.1 Miscellaneous duties are performed according to instruction.
