GENERAL SUMMARY

Renders nursing care and supervision to clients in residence under general supervision of the Program Manager/Supervising Registered Nurse with backup clinical consult of the Division Director.

 

MINIMUM QUALIFICATIONS

Valid and current Florida Licensed Practical Nurse License.

 

PHYSICAL REQUIREMENTS

  • Must be in good physical condition with no restrictions of movement. 
  • Must be able to apply Therapeutic Effective Aggression Management (TEAM) Techniques without limitations.

 

ESSENTIAL JOB FUNCTIONS

 

1.0

Observes and assesses   client's course of treatment while in residence.

 

1.1

Documents clients progress   according to identified needs and goals of the treatment plan.

 

1.2

Administers nursing   care/treatment according to Agency protocol and procedures in accordance with   accepted nursing care standards.

 

1.3

Attend and participates in   daily treatment team meetings as appropriate.

 

1.4

Recognizes and reports any   signs of aggressive behavior.

 

1.5

All clients on special   precautions are monitored according to unit protocol and procedure.

 

 

 

2.0

Administers physician   ordered medications to clients.

 

2.1

Demonstrates the ability to   administer medication and oversee self-administration in accordance with unit   policy and procedure.

 

2.2

Documents medication   education.

 

2.3

Accurately counts,   administers and charts all medications given according to established   procedure.

 

2.4

Verifies original   medication order by client name, medication, dosage, time and route of   administration.

 

2.5

Administers all PRN   medications according to appropriate indications and verifies with original   order.

 

2.6

Administers all scheduled   medications within 60 minutes of time ordered.

 

2.7

Monitors and documents   responses/reactions to medication in accordance with unit protocol and   procedure.

 

2.8

Documents all medication variances   and reports same to Supervising RN.

 

 

 

3.0

Ensures and accepts   responsibility for security of medication at all times.

 

3.1

Medication count is   conducted at end of each shift.

 

3.2

Reports and documents all   medication variations to Supervising RN.

 

 

4.0

Prepares written   documentation for all services rendered.

 

4.1

Written documentation for   services rendered is performed in accordance with the Agency's standards for   service documentation in the client's medical record.

 

4.2

Accurately documents   observations. procedures and care provided during the client's course of   treatment.

 

4.3

All charting is signed with   first initial, last name and title 100% of the time.

 

4.4

Documentation is neat,   legible and in accordance with accepted charting standards.

 

 

 

5.0

Conducts all activities in   such a way as to safeguard the confidential status of client information.

 

5.1

All "on the job" and "off   the job" activities are performed in accordance with the Agency's client   confidentiality standards.

 

 

 

6.0

Admits new clients to the   unit.

 

 

6.1

Ensures charts are   assembled properly.

 

 

6.2

Ensures completeness of   paperwork.

 

 

6.3

Obtains as much general   information from the client and family as possible.

 

 

 

 

 

7.0

Provides general direction   and oversight to MHTs on assigned shift.

 

 

7.1

Consistently motivates MHTs   in a positive manner, develops teamwork in accepting and practicing the   policies and procedures of the Agency.

 

 

7.2

Provides direction as   necessary, always leads by professional example.

 

 

 

 

 

8.0

Participates in unit   meetings, training and staff development activities.

 

 

8.1

Staff meetings are attended   80% of the time.

 

 

8.2

Maintains current LPN   license and knowledge of current nursing developments.

 

 

8.3

Completes all orientations   and mandated training activities according to criteria established for   position.

 

 

8.4

Participates in relevant   staff development activities.

 

 

8.5

Participates in weekly   Treatment Team meetings in the absence of the Lead LPN.

 

 

 

 

10.0

Maintains a good attendance   record.

Notifies supervisor of any   absence 4 hours prior to beginning of shift.

Supervisor is always   notified and approval granted prior to leaving assigned work area.
